I give permission to my child to attend Mount Zion Assembly's Summer Bible Camp (SBC) with the knowledge that my child will have to abide by the guidelines and rules of the SBC. I understand that my child will be encouraged to participate in all the activities such as singing, listening to Bible stories, doing crafts, and playing games with other children.
I understand that Mount Zion Assembly volunteers love and care for my child, and have their safety in mind. However, in the event of an accident where my child may be injured (i.e. falling on the playground), I will not hold Mount Zion Assembly responsible, knowing that they have done whatever possible to prevent such an occurrence.
I understand that I am more than welcome to come and observe the SBC program at any time, and am free to speak to the Mount Zion Assembly volunteers concerning any concerns or questions I might have.
